Members of the SilverSneakers Fitness Program may use the locator at the top left of this page, “Locate a Participating Fitness Center”, to find a facility in their area.

Healthways works directly with fitness centers and providers to build a robust network for our members. The fitness center submission must be completed by the facility. We do not accept, and will not process, information that is submitted by the member. Working directly with the center ensures that we are talking with the key decision makers at the facilities, confirms their interest in our program and allows us to collect all of the necessary information accurately. All of these steps ensure a smooth process in the submission of their interest.

Facilities who would like to notify Healthways of their interest in offering one of our programs, please click “Fitness Center Submission” to the left of the screen. We ask that you complete a brief questionnaire so that we may have your correct information on file.

The SilverSneakers Providers in our Network are chosen based on many factors. These factors include but are not limited to their geographic location, the need to add a facility into the network, the overall quality of the facility, the high level of customer service provided, and accessibility standards.
